Privacy
Your information deserves care.
This policy explains how Sean Fraser Clinical Psychology handles personal information collected through this website, enquiries and the provision of psychology services. It also explains how to request access or correction and raise a privacy concern.
About the practice and this policy
This website is operated for Sean Fraser, Clinical Psychologist, in Windsor, New South Wales. In this policy, “the practice”, “we” and “us” refer to Sean Fraser Clinical Psychology. Privacy enquiries can be sent to sean@sfclinpsych.com.au. We handle personal and health information under the Privacy Act 1988 (Cth), the Australian Privacy Principles and applicable NSW health privacy law.
Clinical information
To arrange and provide psychology services, the practice may need identifying and contact details, date of birth, referral and funding details, relevant health history, assessment information and clinical notes. Information is normally collected from you. With your consent, or where the law permits or requires it, relevant information may also come from a referring practitioner, another treating professional or an authorised representative. Clinical consent and information-sharing arrangements are discussed as part of care.
Service providers
Online booking is provided by Halaxy. Details entered into its booking calendar are submitted directly to Halaxy, rather than through the website enquiry form. Halaxy processes information under its own privacy policy. Its Australian privacy statement says data is stored in Australia, while some disclosures through connected services may occur overseas. This does not mean every integration described by Halaxy is used by this practice.
Website hosting, email, backups, security and technical support services may process enquiry details and technical information as needed to operate and support the website. Access is limited to the purposes of providing those services. External services can have processing or support operations outside Australia; contact Sean for details about the providers and locations relevant to your enquiry or care. Links to other websites, including Google Maps and Halaxy, take you to services with their own privacy arrangements.
Website enquiries
Using the enquiry form sends your email address, optional name and message to Sean by email and saves a copy in the WordPress website database. These details are used to respond to and manage your enquiry. They may also appear in routine website or email backups. Please supply only the information needed for a practical question.
Do not use the form to send detailed health information, referral documents, payment card details or urgent requests. The website form is not a clinical record system. Submitting an enquiry does not confirm an appointment or establish a psychologist–client relationship. If you accidentally send sensitive information, contact Sean to discuss how it should be handled.
You may browse the website without identifying yourself and use a pseudonym for a general enquiry. An email address is needed for a reply. For appointments, care, rebates or funding, identification and other information may be necessary; without it the practice may be unable to provide the requested service.
How information is used and shared
Personal information is used to respond to enquiries, arrange appointments, provide and administer care, communicate about services, manage billing or rebates, and meet legal and professional obligations. Health information is collected with consent unless another lawful basis applies.
Clinical information is confidential, subject to legal exceptions. Relevant information may be shared with your consent, for the purpose for which it was collected or a directly related purpose you would reasonably expect, or where permitted or required by law. This may include communication with a referring or treating practitioner, a funding body or an authorised representative. Disclosure may also be required by a court order or other legal duty, or permitted to address a serious threat to life, health or safety. Only information appropriate to the circumstances should be shared.
An enquiry does not subscribe you to marketing. The website enquiry form has no newsletter signup or marketing-list action.
Security and retention
Enquiry information is held in the website database, the receiving email system and associated backups. Clinical information is handled separately through the practice’s clinical arrangements. Reasonable security safeguards are required for personal and health information; no internet transmission or storage system can be guaranteed completely secure. Please ask Sean for a suitable channel before sending sensitive documents.
Health records collected when a person was an adult must generally be kept for at least seven years after the last health service. Where information was collected before age 18, it must generally be kept until age 25. Other legal, professional or dispute-related requirements may require longer retention. Enquiries and administrative information are retained as needed for their purpose and applicable obligations. Information no longer required should be securely destroyed or de-identified, subject to those obligations and backup cycles.
Access and correction
You may ask to access personal or health information held about you and request correction of information that is inaccurate, incomplete, out of date, irrelevant or misleading. Sean will consider the request under applicable privacy law and explain any lawful reason for refusing access or correction and the available complaint options. If a correction is disputed, you may ask for a statement of your requested correction to be associated with the record.
There is no fee to make an access or correction request. If a lawful, reasonable charge applies to providing access, Sean will explain it in advance. Requests will be handled within the period required by law.
Privacy complaints
Please raise a concern with Sean first, explaining what happened and the outcome you seek. Sean will consider the complaint, make any necessary enquiries and respond within a reasonable period, generally within 30 days. If more time is needed, you will be advised.
If the matter is unresolved, you can contact the Office of the Australian Information Commissioner. For concerns about health information handled by a NSW private health service provider, you can also contact the Information and Privacy Commission NSW. These bodies can explain their complaint requirements and jurisdiction.
Access, correction and privacy enquiries
Contact Sean at sean@sfclinpsych.com.au with the subject “Privacy enquiry” to ask about your information, request access or correction, or make a complaint. Please describe your request without sending identity documents or detailed health information by ordinary email. Sean may need to verify your identity and arrange a suitable secure channel.
